Use this form to provide updates, additional medical records, diagnostic results, or photographs for patients currently under the care of Veterinary Vision. You may also use this form to submit follow-up questions regarding a previously evaluated patient.

Please note that this form is intended only for patients that have already been examined by Veterinary Vision. If you are requesting guidance for a new patient that has not yet been evaluated, please submit an E-Consult or Patient Referral instead.

Once your E-Consult has been submitted, it will be reviewed by one of our doctors during regular business hours.

Recommendations are typically provided within 2–3 business days. Our office is open Monday - Thursday.

To support continuity of care, Veterinary Vision asks that client communication and medication refills be coordinated through the primary veterinary hospital when ongoing monitoring and examinations are being performed there. We are always happy to provide additional guidance to the referring veterinarian as needed.
